Local Filing Requirements
- Civil Docket Cover Sheet.
A civil docket cover sheet, in a form supplied by the clerk, must be completed and submitted with any complaint commencing an action or any notice of removal from state court. Each cover sheet must cite the title and section of the United States Code or relevant statue pursuant to which the action or notice is filed. A cover sheet is for administrative purposes only. - State Court Docket Sheet.
When any notice of removal from state court is filed, the filing party must also attach to the notice of removal a copy of th docket sheet from the circuit court from which the case is being removed in addition to any other document required by federal rule or statue.
